How to Apply

Please note that our service area is the state of Arkansas and eastern Oklahoma.  This is so we can ensure adequate follow-up training and support to teams.

The application process starts with a written application package emailed to swservicedogs@gmail.com or mailed to us at Southwest Service Dogs, Box 321, Clarksville, AR  72830.  You can download the necessary forms here. Both the  Program Application and Medical History Form (to be filled out by the applicant's physician) need to be filled out and returned prior to an interview.

The wait between an application being received and the actual placement will vary because of the importance of the "match." If we have a dog that is ready to graduate that is a match for your personality type, the wait could be very short. Of course, if we don't have an immediate match, the wait might be longer.
